The transition of moving to a care home is not easy and can be particularly challenging for people living with Dementia, who will find changes to familiar routines, or surroundings confusing and frustrating.

We have particular experience in supporting people with Dementia so we understand how to help make the transition as stress-free and reassuring as possible.

This extends to supporting our residents’ family members during the transition and beyond.

We understand that the decision to move someone you love to a care home is a difficult one and we aim to make the process as worry-free as possible.

We welcome your questions – we know you will have lots of things you want to ask us.

We will give you clear, straightforward answers. We will also draw upon your unique knowledge of your loved one so that we can tailor support to their needs.

Visitors are ALWAYS welcome ANY time. That’s why RCH Care Homes are a real home from home – we encourage you to feel free to pop in any time and continue to be fully involved in the life of your loved one.

Personalised care plans

Delivering personalised care, tailored to each individual’s needs, wishes and preferences is at the heart of everything we do, so we take time to get to know each of our residents.

By understanding each person’s life story and interests, as well as physical and emotional needs, we make sure that our residents spend their time in ways that they most enjoy. Our homes offer opportunities to get involved in a wide range of stimulating in-house and external activities.

Wherever possible, we involve our Dementia residents in drawing up their personal care plan and we also consult with family members.
